I have installed Who's Online v 4.1.0, and it seems to work fine, but when I try to view the Modifications Worlets in the CP, all I get is a line that says "All Fields Required" and the button. No worlet fields appear. I did do the Wordlet mods first per instructions. I also re-did the CP.cgi mods again as I saw somewhere here that was recommended for someone else with a similar problem. Did you miss cp_wordlets.cgi?

Well, I thought I had, but I did it again from scratch just to be sure. Still have same problem.
When I Try to view the "Modifications Wordlets" in CP, all I see is the statement "All Fields Required" and the Update button. No wordlets are being displayed.

Ok, I fixed it. The instructions for the Wordlets Mods (the thing you do first) has a typo.
For the cp.cgi it says:
# find:
wordlets_cp wordlets_err
# replace with:
wordlets_cp wordlets_err wordlets_mod
It should say "wordlets_mods" (with an "s" on the end). I added the "s" and it worked!
