from a command line, enter that line, but replace hostname with the host (probably localhost), username with your mysql connection username, database with the name of the database within mysql you want to use, and once you have that connection, import and run the sql commands located within initialize_tables.txt

Yea, one host I was with, I had to connect via SSH, and once I got logged in to the mysql, execute that line (obviously with the correct info) and it should work.
Of course the easier method I find is simply to setup phpMyAdmin (which is quite easy also), and browse to the file and upload it.
